Area Plan (pdf) - Provides an overview of the social, economic and demographic characteristics of the planning and service area. Focus is given to geographic areas and population groups within the PSA that have special needs, specifically as they relate to low-income, minority and rural factors.


ARRA Application (doc) - ElderSource is requesting information from established organizations experienced in providing Elder Nutrition Services in Baker, Clay, Duval, Flagler, Nassau, St. Johns and Volusia Counties to provide Congregate and Home Delivered meal services under the American Recovery and Reinvestment Act Elder Nutrition program (ARRA).

Library Card Application (pdf) - ElderSource operates a Resource Library with the assistance of funding from The Community Foundation. The Resource Library is a lending library of aging related materials that are available to elders, their caregivers, professionals working in the field and service providers.
