  Planning & Preparedness
      For your information, we have gathered together various documents detailing the   Intricacies of our numerous functions as Northeast Florida's Area Agency on Aging.
From accessibility studies to our annual reports, you will find detailed analyses.
 

Area Plan 2009 (pdf) - Provides an overview of the social, economic and demographic characteristics of the planning and service area. Focus is given to geographic areas and population groups within the PSA that have special needs, specifically as they relate to low-income, minority and rural factors.

GIS Map Data Request Form - The maps will be provided in a practical timeframe. In special cases (e.g. custom maps or large orders), we will provide the maps in a time period not to exceed 10 business days unless a different time frame is negotiated during the map request.

Disaster Plan information, and helpful tips and resources - The function of state, regional, and local agencies in disaster preparedness, response, recovery and mitigation procedures is to address and meet the needs of elder citizens through the coordination of mutual assistance.

Needs Assessment - Needs Assessment Survey Area, PSA4 - The PSA includes Baker, Clay, Duval, Flagler, Nassau, St. John’s, and Volusia Counties The needs assessment is critical to the planning process and will provide much-needed empirical data upon which to base the planning efforts of the agency.

Accessibility Study - The purpose of the study was to determine how elders and their caregivers find out about community resources and services, how they access those services, difficulties encountered when attempting to access services, as well as the overall visibility of ElderSource.
