Provides an overview of the social, economic and demographic characteristics of the planning and service area. Focus is
given to geographic areas and population groups within the PSA that have special needs, specifically as they relate to low-income,
minority and rural factors.

The maps will be provided in a practical timeframe. In special cases (e.g. custom maps or large orders), we will
provide the maps in a time period not to exceed 10 business days unless a different time frame is negotiated
during the map request.

The PSA includes Baker, Clay, Duval, Flagler, Nassau, St. John’s, and Volusia Counties The needs assessment is critical to the planning
process and will provide much-needed empirical data upon which to base the planning efforts of the agency.

access those services, difficulties encountered when attempting to access services, as well as the overall visibility of ElderSource.
