  Be Listed as a Resource in Our Database!
 
      The Elder Helpline uses a robust resource database to provide elders and their
   caregivers information about resources in the community and to assist them with referrals.
 
Policy:
If you offer a service or resource for elders or caregivers and wanted to be listed in the database, review the inclusion/exclusion criteria, complete the form and return it to Elder Helpline at ElderSource,
4160 Woodcock Drive, Jacksonville, Florida, 32207. If you have any questions about becoming a
resource in the database, call the Elder Helpline at 904-391-6699 or 1-888-242-4464.
