ElderSource is requesting information from established organizations experienced in
providing case management and administrative functions for home and community based
services for the elderly in Baker, Clay, Duval, Flagler, Nassau, St. Johns and Volusia
Counties. State grant funds are provided to designated Lead Agencies through the
Community Care for the Elderly (CCE), Alzheimer’s Disease Initiative (ADI), Home
Care for the Elderly (HCE) and Medicaid Waiver programs.
The contract period is from July 1, 2010 through June 30, 2011 and may be renewed annually for up to five additional years.
